Step 2: On-Boarding with the TP Enablement Team

You huddle with the TeamPatient Enablement Team to discuss your goals/priorities and plan your Journey.

  • What is your “Big Goal” – where are you in your journey? Your goal is where you want to be.
    • Define your top priorities.  A manageable number is anywhere from 1 to 5.
  • Name your Journey Team
    • Champion
    • Other parties you want involved in your Journey
  • Create your Groups: Groups enable you to bring together the people involved to address your top priorities in your Journey – e.g. treatments and medications, visits, tests, etc.
    • Purpose
    • Members
    • Documents/Resources
    • To Dos

Important Note: The Groups you create are specific to where you are in your journey.  They change as you move through the journey and the membership of your Team may also change over time.

Helpful Information
  1. Your Team:
    • Champion – he/she is a trusted party; he/she is someone that can be objective, that you trust to be your journey buddy. They will help you keep your journey on track; help you move toward your goals by helping manage the details. This is a must have!!
    • Family – members of your family that you want to be part of the Team.  The degree of involvement may vary from “keep updated on status” to active participation in your journey
    • Friends – The degree of involvement may vary from “keep updated on status” to active participation in your journey
    • Medical Team – doctors, oncologists, other specialists
    • Wellness Team – wellness specialists, reiki, yoga, etc.
  2. Documentation
    • Test results, after visit summaries, doctors’ orders, prescriptions, etc
